Never rely on your client to get a complete list of his medical providers and treatment! Clients cannot be trusted when it comes to their medical history (yes, some of them will even withhold the truth from you).

The only way to ensure that you get a complete list of medical records is the itemized billing statement of your client’s health insurer. Upon request, your client’s health insurer will give you an itemized list of every medical invoice that it has paid, the date of the payment and insurance codes indicating the type of medical treatment rendered.  You should check the health insurer’s billing statement against the medical records you possess to find out if you are missing any records.

In cases where your client’s medical care is paid by Medicaid, the billing statement is called a “Claim Detail Report” and it will be provided by the Department of Social Services in your client’s county of residence.  When Medicare is paying for medical treatment, the itemized list of medical payments is called a “Conditional Payment” letter.
